Reporting for the Local Report Card
The Local Report Card measure will be based on data submitted by districts for each of the four components above.
What will Local Report Cards show? For the daily physical activity, BMI and compliance with local wellness policy components, Local Report Cards will display a “yes” or “no” to reflect whether or not districts reported data for these activities.
How will districts report information for this measure? On this Web page, see the reporting forms for daily physical activity (which comes to ODE) and BMI screening tools (which are sent to the Ohio Department of Health). Districts may respond to questions about the Local Wellness Policy component through EMIS during the N reporting period. BMI SCREENING
A Body Mass Index (BMI) waiver is no longer required if your district elects not to conduct BMI screening. New legislation within Senate Bill 316 changed the language in Ohio Revised Code making the implementation of the BMI screening optional, thereby eliminating the need to obtain a state waiver. Districts that elect to conduct BMI screening will continue to submit their data to the Ohio Department of Health at School.BMI@odh.ohio.gov. Participation in BMI screening will be reported on the District Physical Education and Wellness Measure on the Local Report Card beginning with the 2012-13 school year. Implementation Steps
BMI Screening Data Submission; Recent Change in Tools
There have been several minor modifications to the BMI Collection and Submission Tools. On the collection tool, the CDC BMI submission tool is mentioned as an alternate tool; variables to be collected/submitted were made clearer; the date for submission to the district was deleted (this should be determined by the district). On the Submission Tool, changes include the addition of IRN and school year entries and a note to send as a worksheet, not a scanned copy or a PDF document.
If BMI screenings and data collection have been completed already and/or submitted for the 2011-2012 school year, the new forms should be used the following years. All screening data must be submitted no later than June 1 of each year.
- School Building BMI Collection Tool (Excel)
School building personnel can use this tool for data recording and for submission to the district office. Do not send this workbook to the Ohio Department of Health. If your school is a chartered nonpublic school, please send to the Ohio Department of Health only the District Submission Worksheet found in this workbook.
- District BMI Submission Tool (Excel)
District office personnel can use this tool to combine school building data for submission to the Ohio Department of Health. This is the only worksheet to be submitted to the Ohio Department of Health, and the worksheet can be submitting only via email to School.BMI@odh.ohio.gov. ODH cannot accept information other than grade level, sex and the BMI percentile for each student.
